Obtain Authorisation No.

Contact your medical aid or insurance company to obtain your authorisation number.

Complete Forms

Complete the hospital admission form and anaesthetist consent form.

Submit Forms

ON THE DAY OF
ADMISSION

Present your ID document and medical aid/insurance card. Patients that are not South African citizens should bring their passports along.

PLEASE NOTE: JHB Eye Hospital is a facility used by Doctors to perform operations and NO consultations take place at JHB Eye. The doctors affiliated with Johannesburg Eye Hospital each have their own independent practices and consultations are conducted in their rooms and not at JHB Eye. Therefore unfortunately JHB Eye hospital is not able to assist with booking appointments.
